To avoid delays in your claim, here’s what you need:
What happened to your device
To file your claim we’ll need to know the exact date of incident, the last time you used your device and whether it was dropped, got wet, is malfunctioning or something else happened.

Your device information
We need to validate your registered device as being covered under your policy so we will need your purchase receipt or proof of ownership that identifies the device by it's IMEI or Serial Number.
You can also get this information from your carrier
You will need to fill out and upload some simple forms to verify your identity.
Your assigned claims representative will forward the forms to you shortly after your claim is submitted.
Your deductible is only a fraction of the full retail price of your device and you can pay it easily online by credit, debit, or prepaid card. Once your claim is approved we will get you back up and running usually within a few business days depending on the circumstances of your claim.
